29 CFR 1910.140(c)(13)(ii): Anchorages were not designed, installed, and used, under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two: On 11/16/2021, while in the process of cleaning gutters at a residential property located at 412 Pioneer Springs Ranch Rd. in Aspen, CO, employees were not protected from a fall of approximately 40 feet by conventional fall protection and/or devices. Although vertical lifelines with lanyards and harnesses were provided; the employer failed to ensure that anchors used as a base were installed and used under the supervision of a qualified person. While utilizing a vertical lifeline, an employee attached to a satellite television dish instead of roof anchor.
