29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 11/17/2021, five employees at the rear of the church at 72 Main Street Ellington, CT, were exposed to approximately 20' falls and 30' falls: a. The ladder from the ground to the 20-foot landing did not extend at least three feet above the landing surface. b. The ladder from the 20-foot landing to the east side of the building did not extend at least three feet above the upper roof of the church, 30 feet above the ground. c. The ladder from the 20-foot landing to the west side of the building did not extend at least three feet above the upper roof of the church, 30 feet above the ground.
