1926.502(h)(1)(v)  The safety monitor shall not have other responsibilities which could take the monitor's attention from the monitoring function. On or about November 17, 2021, during the installation of roofing material for a Church located at 3431 Superior Ave, Cleveland, OH 44114; the employer did not ensure employees working from a low-sloped roof were protected from a fall of approximately 21 feet by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.
