29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) Worksite; 1st Years Daycare LLC, 925 Franklin Avenue, Pittsburgh PA - On or about November 17, 2021 : Employees were engaged in shingle roof replacement while walking on a steep 10 x 12 slope roof which was approximately 45 feet above ground level.  Employees did not use a personal fall arrest safety system, nor were they protected by guardrail systems or an alternate fall protective measure.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  a) Worksite; 1st Years Daycare, LLC, 925 Franklin Avenue, Pittsburgh PA , On or about November 17, 2021- The employer has not provided a training program to employees who were tasked with using a personal fall arrest system.     Employees were engaged in shingle roof replacement while walking on a steep 10 x 12 slope roof which was approximately 45 feet above ground level.
