Employees did not use a personal fall arrest safety system, nor were they protected by guardrail systems or an alternate fall protective measure.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: a) Worksite; 1st Years Daycare, LLC, 925 Franklin Avenue, Pittsburgh PA , On or about November 17, 2021- The employer has not provided a training program to employees who were tasked with using a personal fall arrest system. Employees were engaged in shingle roof replacement while walking on a steep 10 x 12 slope roof which was approximately 45 feet above ground level. Employees did not use a personal fall arrest system, safety net system, nor were they protected by guardrail systems or an alternate fall protective measure.
