29 CFR  1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:   On or about November 17, 2021, the employer failed to ensure that their written machine specific procedures for the application of Lock Out Tag Out included checking to ensure that stored energy was released prior to verifying that all energy sources were inoperative and properly locked and tagged out.

29 CFR  1910.147(d)(5)(i): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe.   On or about November 17, 2021, the employer failed to ensure that their written machine specific procedures for the application of Lock Out Tag Out included checking to ensure that stored energy was released prior to verifying that all energy sources were inoperative to ensure all sources were properly locked and tagged out.

29 CFR  1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:  On or about November 17, 2021, the employer failed to ensure that their written Control of Hazardous Energy Program for Lock Out Tag Out was reviewed at least annually to ensure it is up to date and meets all of the requirements of the Standard to include employee audits.

Employees are exposed to possible struck by hazards if one of the slings falter when in use.

On or about November 17, 2021, the employer failed to ensure that the nylon slings being used with the overhead 3 ton yellow Yale crane were in good condition in that they were free of excessive dirt, rips and tears and had legible tags in place to depict the weight limits and requirements.  Employees are exposed to possible struck by hazards if one of the slings falter when in use or if one is used over its capacity limits.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:  a)  On or about November 17, 2021, the employer failed to ensure that effective guarding was in place on the punch presses, including but not limited to press #240 (50 presses in total).  Employees are exposed to struck by and point of operation hazards in that the mesh guarding in place only covers a portion of the front of the machine.  There are varying  openings  where hands and fingers can come in to contact with the press as its activated by the foot pedal.  b)  On or about November 17, 2021, the employer failed to ensure that effective guarding was in place on the drill press #42, yellow top .  The drill spindle, chuck and bit do not have a guard in place to protect employees from struck by and point of operation hazards when operating the drill.      NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1422697, citation 1, item 4, which was affirmed as final order on October 30, 2019 with respect to the same workplace location as this current inspection.
