29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a) At the jobsite, employees used two 24-foot extension ladders to gain access to the rooftop and the ladder were not extended three feet about the landing, exposing the employees to a fall hazard of approximately 20 feet.
