Alleged Violation Description	29 CFR 1926.451(f)(15): Ladders shall not be used on scaffolds to increase the working level height of employees.  a) Jobsite 51 Chestnut Street, Gardner, MA: On or about November 11, 2021, an employee was observed utilizing a folded A frame ladder which had been placed on top of the deck of a pump jack scaffold in order to gain an increase in working level height.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder.    a) Jobsite 51 Chestnut Street, Gardner, MA: On or about November 11, 2021, employees accessing a pump jack scaffold from an aluminum extension ladder were not protected from a fall hazards because the ladder did not extended at least 36 inches above the landing surface nor had grab rails and lashing of the risers been provided instead.
