29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in an in-patient hospitalization, amputation or the loss of an eye. a) K.M.U Trucking & Excavating, LLC on or about October 21, 2021 at the address of 3250 East River Road Sheffield Village, OH: The employer failed to report the hospitalization of an employee as a result of a workplace injury/incident. The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of an employee's hospitalization within 24 hours of being hospitalized. The employer later contacted the OSHA Office on November 16, 2021.
