On or about November 18, 2021, at the above addressed jobsite, employees performing masonry operations, from a tubular welded frame scaffold, at approximately thirty-two (32) feet above the ground, were exposed to fall hazards due to the lack of end-rails.

29 CFR 1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a. On or about November 18, 2021, at the above addressed jobsite, an employee working on the ground, below roofing activities, was not wearing a hard hat to protect against falling materials and debris. Stanley Brickwork, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1233000, citation number 1, item number 1 and was affirmed as a final order on September 7, 2017, with respect to a workplace located at 2341 W. Walton St., Chicago, IL 60622.
