29 CFR  1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, had not been conducted as required by paragraph (l)(4)(ii) to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely:  (a) At the workplace: On or about November 16, 2021, the employer did not provide the forklift operator with refresher training and evaluation.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) On the jobsite: On or about November 16, 2021, employees were exposed to an 18ft-7in fall hazard when performing roofing work without fall protection.
