29 CFR  1910.176(b):Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. On or about November 18, 2021, at this location, employees were instructed to load pipes onto a trailer flatbed that did not have blocking to secure load exposing employees to struck by hazards.

29 CFR  1910.178(l)(1)(i):The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). At this worksite, a forklift is necessary to perform the job tasks of loading and unloading materials that are to be shipped. The forklift operator was not certified to operate the forklift and this exposed employees to various hazards.
