29 CFR 1926.502(d)(15)(i): The Employer did not maintain a safety factor of at least two as part of a complete fall arrest system: (a) Location - 835 Western Avenue, Lynn, MA On or about 11/19/2021, employees working more than 25 feet above the ground were attached to a reusable roof anchor on the roof, that was not capable of supporting at least 5,000 pounds due to it being installed with more than 1 employee attached to the anchor.
