29 CFR  1926.100(a): The employer did not ensure that each affected employee working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about November 16, 2021, at jobsite located in 411 Arlington Ave., Elmhurst, IL 60126, an employee engaged in roofing activities was working below the other employees and exposed to struck-by hazards from falling objects.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR  1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about November 16, 2021, at jobsite located in 441 Arlington Ave., Elmhurst, IL 60126 , employees were exposed to eye injuries while engaged in roofing activities operating a pneumatic nail gun. Eye protection was not being utilized while employees engaged in this activity.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On or about November 16, 2021 at jobsite located in 441 Arlington Ave ., Elmhurst,  IL 60126, employees were exposed to fall hazards in excess of 18 feet while engaged in residential roofing activities. Fall protection was not being utilized adequately while performing roofing activities.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about November 16, 2021, at jobsite located in 441 Arlington Ave., Elmhurst, IL 60126 , employees performing roofing activities were exposed to fall hazards in excess of 18 feet with inadequate protection when the employer failed to ensure they were adequately trained to recognize and minimize the hazards associated with fall.
