29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  A) On or about November 19, 2021, the automatic MM01 and MM03 milling machines, did not utilize guarding methods at the point of operation to prevent employees from reaching the rotating cutting wheel on the mills.  B) On or about November 19, 2021, the front of the Enerpac 10 ton yellow hydraulic assembly press, did not utilize guarding methods to prevent employees from reaching into the point of operation of the press while using single hand lever or foot pedal.   C) On or about November 19, 2021, on the Clausing multi-head drill press, the Clausing 20 did not utilize guarding methods to prevent employees from getting caught in the drill bit, chuck or spindle while operating the drill press.   D) On or about November 19, 2021, on the TL02 and TL19 lathes there was no chuck guard in place to prevent employees from getting caught in rotating parts.   E) On or about November 19, 2021, the deburring tools on the screw machines, such as but not limited to ACME screw machines A27 and A35 did not utilize guarding methods to prevent employees from getting caught in the rotating deburring tool.

29 CFR  1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) On or about November 19, 2021, the employer did not have an energy control program for Carmichael Tool Machine that included training for authorized, affected, and other employees, and periodic inspections of the procedures on an annual basis for equipment such as but not limited to; milling machines, horizontal and vertical lathes, presses and screw machines.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (1) turn off and shut down the machines, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric and pneumatic energy sources, (3) affix lockout or tagout devices to each energy isolating device, (4) verify effectiveness of energy isolation.  a) On or about November 19, 2021, the employer did not ensure that employees, such as but not limited to machines operators, leads and supervisors who were performing servicing and maintenance on machines, such as but not limited to the TL lathes did not lock out the energy sources (electric) when performing tasks such as changing collets on the lathes.
