29 CFR  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were not protected from crushed by hazards by not having operator restraint devices installed on powered industrial equipment:  a. 1 Silverline Dr., North Brunswick, NJ - Warehouse  The employer did not ensure that employees operating powered industrial equipment used in the movement of materials within the warehouse used operator restraint devices.  A mechanic operated forklifts that included, but were not limited to, Komatsu, Model FG25ST-12 forklifts, and did not use seatbelts that were intended to assist the operator in reducing the risk of entrapment of the operator's head and/or torso between the truck and ground in the event of a tip over.  Violation observed on or about November 19, 2021  b. 1 Silverline Dr., North Brunswick, NJ - Warehouse  The employer did not ensure that employees operating powered industrial equipment used in the movement of materials within the warehouse used operator restraint devices. 29 CFR 1910.37(a)(1): Exit routes shall not have flammable, explosive, combustible gases or flammable furnishings stored in the exit route.  a. 1 Silverline Dr., North Brunswick, NJ - Production area - #1 Contour Intergrid  The employer did not ensure exit routes were maintained free of flammable/combustible gas.  The employer installed and maintained flammable storage lockers containing flammables, and a 55 gallon drum of Isopropyl Alcohol,   directly in the exit route from the production area, at or near Contour Intergrid, to an outside exit.  Violation observed on or about November 19, 2021  b. 1 Silverline Dr., North Brunswick, NJ - Shipping Dock Exit   The employer did not ensure exit routes were maintained free of flammable/combustible gas.  Employees engaged in the manufacture of vinyl window products and warehousing activities placed empty propane bottles from the powered industrial equipment at the emergency exit door.  Employees used propane bottles as a door stop to keep the exit door open.  Violation observed on or about November 19, 2021
