The employer did not render such means inoperable and then ensure that such means were tagged to indicate that employees were at work.  a) Bethsaida Road Electrical Substation: On or about November 16, 2021, the employer exposed an employee to electric shock and burns from arc flashes, in that the employer did not ensure all sources of electrical energy that may be supplied to the particular lines and equipment were open while employees perform work including, but not limited to, taking measurements for electrical bus framing.
