29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Francisco Talavera located at 6448 Meadow Glen Drive, Westerville, OH 43082:  On and preceding November 22, 2021, the employer did not ensure that three extension ladders used for access to different sections of the roof of a multi-story residential home had the side rails extending at least 3 feet beyond the upper landing nor were the ladders secured the top with a grasping device provided. The following condition exposed employees to falls up to 18 feet.

29 CFR  1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Francisco Talavera located at 6448 Meadow Glen Drive, Westerville, OH 43082:  On and preceding November 22, 2021,  the employer did not initiate and maintain safety and health programs for employees installing roof shingles, thereby exposing employees to the hazards associated with performing roofing work.    Francisco Talavera was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1335312, citation number 1, item number 1 and was affirmed as a final order on September 20, 2018, with respect to a workplace located at 10542 Elderberry Drive (Lot #20) in Plain City, Ohio 43064.

Francisco Talavera was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1335312, citation number 1, item number 3 and was affirmed as a final order on September 20, 2018, with respect to a workplace located at 10542 Elderberry Drive (Lot #20) in Plain City, Ohio 43064.

Francisco Talavera was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1335312, citation number 1, item number 4 and was affirmed as a final order on September 20, 2018, with respect to a workplace located at 10542 Elderberry Drive (Lot #20) in Plain City, Ohio 43064.
