29 CFR 1910.219(d)(1):Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): a) GE Lighting Inc. on or about November 22, 2021 at the address of 1250 S. Walnut St. Bucyrus, OH: The employer did not ensure the power transmitting belt and pulley(s) of the direct drive motor for the "Bulb crusher" HS1 coater conveyer was adequately guarded. Following a maintenance and cleaning operation, an employee working on or near this unguarded nip-point was exposed to amputation hazards.
