29 CFR 1910.219(c)(4)(i):Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. a) On or before 11/16/2021 the employer did not ensure that a 1 1/4 inch protruding rotating shaft for the Line 1 Down Cutter located at ground level was properly guarded by a nonrotating cap or safety sleeve.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section. a) On 11/16//21 the employer did not ensure that the approximately 3" gap between the conveyor belt and the framing of the Line 1 Down Cutter, that was at ground level, was properly guarded in that employees were exposed to the moving conveyor belt while clearing debris from in-between the belt and the framing of the machine.
