29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about November 23, 2021, an employee operating the Box Stitcher machine was exposed to the point of operation hazard in that the machine was not equipped with a guard device. b) On or about November 23, 2021, an employee operating the Eberl� off-line core cutter, was exposed to pinch point hazards created by the feed table articulating while setting tubes to be cut. c) On or about November 23, 2021, an employee operating the  Eberl� off-line core cutter, was exposed to point of operations hazards in that the machine was equipped with an inadequate guard that did not prevent employees access to the point of operation.  d) On or about November 23, 2021, employees operating the Eberl� Sprial Winder machine on line one, were exposed to ingoing nip points and rotating parts hazards in that the machine was equipped with an inadequate guard that did not prevent employees access to the rubber winder.  e)On or about November 23, 2021, employees operating the Eberl� feed saw on line one, were exposed to point of operation hazards in that the machine was equipped with an inadequate guard that did not prevent employees from accessing the point of operation.  f) On or about November 23, 2021, employees operating the Eberl� Sprial Winder machine on line two, were exposed to ingoing nip points and rotating parts hazards in that the machine was equipped with an inadequate guard that did not prevent employees access to the rubber winder. g) On or about November 23, 2021, employees operating the PACO cut-off saw on line two, were exposed to point of operation hazards in that the machine was equipped with an inadequate guard that did not prevent employees from accessing the point of operation.

29 CFR  1910.219(e)(3)(i): Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs (m) and (o) of this section.   a) On or about November 23, 2021, an employee operating the Box Stitcher machine was exposed to a caught by hazard from the machine's vertical drive belt in that it was not equipped with a guard.

29 CFR  1910.219(f)(1):Gears. Gears shall be guarded in accordance with one of the following methods: (f)(1)(i) By a complete enclosure; or (f)(1)(ii) By a standard guard as described in paragraph (o) of this section, at least seven (7) feet high extending six (6) inches above the mesh point of the gears; or (f)(1)(iii) By a band guard covering the face of gear and having flanges extended inward beyond the root of the teeth on the exposed side or sides. Where any portion of the train of gears guarded by a band guard is less than six (6) feet from the floor a disk guard or a complete enclosure to the height of six (6) feet shall be required.  a) On or about November 23, 2021, employees operating the PACO cut-off saw on line two, were exposed to caught by hazards in that the machine's power transmission guard for the gears had been removed.

29 CFR  1910.305(g)(2)(ii): Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.  a) On or about November 23, 2021, the employer did not ensure that flexible extension cords were not used with splices or taps in that the Box Sticher Machine had been repaired and was being powered by an orange flexible extension cord spliced to the machine.
