29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Gas Field Services LLC located at 255 Arrowhead Road, Little Hocking, OH 45742: On and Preceding November 23, 2021, the employer did not ensure that flexible cords being used to power lighting and a diesel exhaust fluid (DEF) pump were used in accordance with instructions included in the listing or labeling. A flexible cord was plugged in series into another flexible cord in between the wall outlet providing power to the light and the pump. The following condition exposed employees to electrical/fire hazards.
