The employer did not properly evaluate the respiratory hazards in the molding department exposing employee to Carbon Monoxide.

29 CFR 1910.1000(a)(2): Employees were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the 8-hour Time-Weighted Average concentration of 50 parts per million particles of air (50 ppm) for any 8-hour work shift of a 40-hour work week and attenuated accordingly for extended work shifts in extended work weeks: Location; Molding Department, Maintenance office a) An employee was scheduling work orders under a heater that was leaking carbon monoxide. The employee went to the molding dept. to conduct maintenance work on a tube oven (CB 3) that was leaking Carbon Monoxide. The employee was exposed to 191.5 ppm of Carbon Monoxide which was 3.83 times over the OSHA permissible exposure limit of 50 pp during a 330 minute occupational exposure on 11/16/21.

The employee was exposed to 191.5 ppm of Carbon Monoxide which was 3.83 times over the OSHA permissible exposure limit of 50 pp during a 330 minute occupational exposure on 11/16/21. The employer did not implement adequate engineering or administrative controls exposing employees to Carbon Monoxide.
