29 CFR  1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  Employees were exposed to serious caught-in, crushing and or struck-by injuries, when operating including but not limited to the CAT forklift Model # GP30N, Serial # AT13G02921, in that the forklift operators had not completed a competent training program.

29 CFR  1910.1200(e)(1):Employer had not developed or implemented a written hazard communication program which at included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):  At a worksite located at 701 E Gaines Dr. Clinton, Missouri. Employees were exposed to serious health hazards, in that the employer had not developed a Hazard Communications Program for the various types of chemicals in use in the workplace.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Warehouse:  Employees had not received training on the chemicals they were working with at the time of their initial assignment, such as, but not limited to the DO-IT All Purpose and Premium Paints, which contain acetone, xylene and are highly flammable. which is harmful to the lungs and eyes if used without PPE.(Personal Protective Equipment) b) Paint Mixing Room : Employees had not received training on the chemicals they were working with at the time of their initial assignment, such as, but not limited to GEOSHADES Colorant which contains Titanium Dioxide - 2B - Talc - 3 - Amorphous Silica. which is harmful to the lungs and eyes if used without PPE.(Personal Protective Equipment) c) Retail Store : Employees had not received training on the chemicals they were working with at the time of their initial assignment, such as, but not limited to Sunny Side brand Acetone which is highly flammable and harmful to skin and eyes if used without proper PPE. 29 CFR  1904.40(a):Basic requirement. When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours. R.P Lumber Co., Inc.,  did not provide authorized government representative, an OSHA safety compliance officer, conducting an employer reported injury, the requested  300/ 300A logs for years 2016-2020 and any recordable for 2021, within four (4) hours.
