29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or before November 22, 2021, guarding to prevent employees from having any part (s) of their body near the rotating parts of the standing S-lock roll former (model # C-8600-Series), located in the northeast corner of the production area was not affix to/in place on the machinery. An employee in the process of feeding sheet metal through roll former after making an adjustment was injured when his right hand inadvertently contacted the cylindrical roller(s) of the machinery.
