29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 6 Cherry Ln Dr. Englewood, CO 80113: On or about November 23, 2021 and at times prior, employees were exposed to approximately 10-20 feet fall hazards while working on the third story roof and the second story flat roof area of a residential house. The employees were installing sheet metal on the roof without fall protection.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 6 Cherry Lane, Englewood, CO, 80113: On November 23, 2021, and at times prior, employees were exposed to fall hazards of at least six feet to the flat roof below while utilizing a portable ladder to access the third story roof of the residential house to install metal roof panels, without fall protection.
