29 CFR  1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form.   (a) On or about January 19, 2022,  the employer did not identify and evaluate the respiratory hazard(s) associated with employee(s) working at the  low pressure aluminum casting machine.  An employee working at the low pressure aluminum casting machine, was exposed to an airborne concentration of suspected Coal Tar Pitch Volatiles (CTPV) at an 8-hour time weighted average of 0.279 milligrams per cubic meter of air (mg/m^3, approximately 1.57 times the limit of 0.2 milligrams per cubic meter of air (mg/m^3).  The sample was collected on January 19, 2022, for the first shift, during a 425 minute sampling period and exposure calculations include a zero increment for the 55 minutes not sampled.
