On or about 11/24/2021, at the site of 88-12 73rd Ave. Fresh Meadows, NY Employees working on the ground were in danger of head injury from falling objects such as, but not limited to tools and materials from other employees who on the pitch roof. Protective helmets were not worn.

On or about 11/24/2021, at the site of 88-12 73rd Ave. Fresh Meadows, NY Employees installing shingles on a pitch roof were exposed to the hazard of falling approximately 12 ft. to the ground below. The employees were not tied off. Other employees were not provided with personal fall arrest systems.
