29 CFR  1910.37(b)(2): Each exit is not clearly visible and marked by a sign reading "Exit."  On or about 11/24/2021, the employer did not have clearly visible and marked exit signs placed on exit doors of the fabrication and manufacturing building  of Kelpro, Inc. located at 2047 State Hwy Z 63801.   Abatement documentation is required for this violation.

29 CFR  1910.147(c)(1): Energy control program. The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   On 11/24/2021, Kelpro Inc. had not established a program consisting of energy control procedures, employee training and periodic inspections.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about 11/17/2021, an employee received a finger amputation due to lack of guarding.  No barrier guards or electronic device in place to insure that an employee does not exposed body parts to potential caught in points of the Alpine Pipe Bending machine (Model AB 89NCL, S/N 210314).  Abatement documentation is required for this violation.

29 CFR  1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE).  On or about 11/24/2021, a personal protective equipment assessment was not conducted and certified, a program was not implemented.  The Alpine Pipe Bender (Model AB 89NCL, Serial/N-210314) has been installed since last inspection (1351151) conducted on 10/1/2018.    Kelpro was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(d)(1), which was contained in OSHA inspection 1233549, citation number 1, Item 1 and was affirmed as a final order on 6/29/2017, with respect to a workplace located at 2047 State Hwy Z, Sikeston, MO 63801  Abatement documentation is required for this violation.
