The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. 29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about 11/17/2021, an employee received a finger amputation due to lack of guarding. No barrier guards or electronic device in place to insure that an employee does not exposed body parts to potential caught in points of the Alpine Pipe Bending machine (Model AB 89NCL, S/N 210314). Abatement documentation is required for this violation.

Kelpro was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(d)(1), which was contained in OSHA inspection 1233549, citation number 1, Item 1 and was affirmed as a final order on 6/29/2017, with respect to a workplace located at 2047 State Hwy Z, Sikeston, MO 63801 Abatement documentation is required for this violation.
