29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Work site, 3 Main Street, Stafford Springs, CT: On or about November 29, 2021, three employees were exposed to head injuries due to not wearing hardhats while working in proximity to an area where roofing material was falling from heights greater than 30 feet.

29 CFR  1926.416(a)(1):Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  Jobsite located at 3 Main Street, Stafford Springs, Connecticut:  On, or about, November 29, 2021, roofing contractor employees operated an aerial lift basket within 10 feet of a primary electrical transmission line with an estimated voltage of 23,000 volts.
