29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about November 30, 2021, on the north side of the site, employees were utilizing Werner twenty-four-foot extension ladders to gain access to the roof of the project. The ladder was not extended three (3) feet above the upper landing surface and not secured at any point.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about November 30, 2021, on the north side of the project site, employees were performing roofing work was on a commercial roof above six (6) feet with an unprotected side without appropriate fall protection at heights above six (6) feet. CM Metals, LLC was previously cited for a violation of this occupational safety and health standard 1926.501(b)(1), which was contained in OSHA inspection number 1494255, citation number 1, item number 2, with a citation issued October 22, 2020, as a final order on November 04, 2020, with respect to a workplace located at 14363 LA-73, Prairieville, LA, 70769. CM Metals, LLC was previously cited for a violation of this occupational safety and health standard 1926.501(b)(1), which was contained in OSHA inspection number 1494255, citation number 1, item number 2, with a citation issued May 5, 2021, with respect to a workplace located at 17535 Airline Hwy, Baton Rouge, LA, 70716.
