29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: The employer is failing to protect employees from fall and cave-in hazards because there wasn't a ladder located in the trench where employees were working - This was most recently documented on or about November 29th, 2021 at 189th and hazelnut, Gretna, NE 68028. The dimensions of the trench where the employee was exposed to fall/cave-in hazards was approximately 48 inches wide and 4'4'' deep.
