29 CFR 1910.178(a)(4): Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: a) Front Floor - On or about 11/19/2021, an employee was operating a Yale forklift (SN B875V10271K) with 96" forks installed. The forklift's data plate listed capacities for 42" and 48" forks and was not updated for the installed forks. The condition exposed employees to struck-by and caught-between hazards.
