29 CFR  1910.213(r)(3): The point of operation on combination or universal woodworking machines was not guarded as required for such a tool in a separate machine:   (a)Ashley's Cabinets, Inc. at 27063 Henry Place, Sioux Falls, SD 57108:   On or about and at times prior to November 30, 2021, the employer did not ensure the point of operation on the PMK routing machine (model# C-1205) was properly guarded to prevent employees from having their fingers in the danger zone during machine operations. The existing guard did not provide protection from a crushing or amputation hazard in that the distance from the table to the bottom guard was 1 1/8th inch and the face of the guard to the pinch point hazard measured 15/16 of an inch. This routing machine foot treadle was located directly below the point of operation, putting the employees in close proximity to the hazard. The lack of effective guarding or stand-off distance exposed the employees to an amputation hazard.
