29 CFR  1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:   (a)Ashley's Cabinets, Inc at 27063 Henry Place, Sioux Falls, SD 57108:   On or about and at times prior to November 30, 2021 the employer did not ensure work areas were kept clean and orderly in that excess sawdust, and chemical containers were allowed to accumulate in an excess unsafe manner throughout the door manufacturing department and paint department.  This condition exposed employees to slipping and tripping hazards.

29 CFR  1910.107(c)(5): Electrical equipment was not approved for locations containing both deposits of readily ignitable residues and explosive vapors were present in spraying area(s):  (a)Ashley's Cabinets, Inc at 27063 Henry Place, Sioux Falls, SD 57108:   On or about and at times prior to November 30, 2021, the employer did not ensure employees were protected from fire and explosion hazards. A 480-volt ventilation power panel was in use while located right above the work bench the paint department employees use to mix all paints in this area. Paint residue generated from months of overspray accumulated in excess of one eighth of an inch on this power panel, conduit, and associated gang plug outlet receptacles.  This condition exposed employees to fire and explosion hazards.

This condition exposed employees to fire and explosion hazards.

29 CFR  1910.213(r)(3): The point of operation on combination or universal woodworking machines was not guarded as required for such a tool in a separate machine:   (a)Ashley's Cabinets, Inc. at 27063 Henry Place, Sioux Falls, SD 57108:   On or about and at times prior to November 30, 2021, the employer did not ensure the point of operation on the PMK routing machine (model# C-1205) was properly guarded to prevent employees from having their fingers in the danger zone during machine operations. The existing guard did not provide protection from a crushing or amputation hazard in that the distance from the table to the bottom guard was 1 1/8th inch and the face of the guard to the pinch point hazard measured 15/16 of an inch. This routing machine foot treadle was located directly below the point of operation, putting the employees in close proximity to the hazard. The lack of effective guarding or stand-off distance exposed the employees to an amputation hazard.

29 CFR  1910.303(b)(7)(i):Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:  (a)Ashley's Cabinets, Inc at 27063 Henry Place, Sioux Falls, SD 57108:   On or about and at times prior to November 30, 2021, the employer did not ensure that electrical equipment was effectively closed exposing employees to electrical shock and fire hazards associated with one electrical junction box located just above a workbench on the east wall in the cabinet assembly department .  This condition exposed employees to electrical and fire hazards associated with missing knockouts and exposed wiring on the electrical box.
