On or about December 1, 2021, at jobsite located in 644 Saylor Ave. Elmhurst, IL 60126, employees were exposed to eye injuries while engaged in roofing activities operating a pneumatic nail gun. Eye protection was not being utilized while employees engaged in this activity. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: On or about December 1, 2021, at jobsite located in 644 Saylor Ave. Elmhurst, IL 60126, employees performing roofing work at a residential property were exposed to fall hazards in excess of 12 feet with no fall protection when the employer failed to ensure they were adequately trained to recognize and minimize the hazards associated with falls. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
