29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about November 30, 2021 at least two employees working on the second story roof were not protected from a fall hazard of approximately nineteen (19) feet; (b) On or about November 30, 2021 at least two employees working on the first story roof were not protected from a fall hazard of approximately nine feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about November 30, 2021, four employees using four extension ladders were exposed to a fall hazard: (a) An aluminum extension ladder placed at the front of the house to access the second level roof had its side rails approximately eighteen (18) inches past the roofline exposing employees using it to a fall of approximately nineteen feet; (b) A fiberglass extension ladder placed on the side of the house with its side rails approximately six inches past the roofline exposing employees using it to a fall hazard of approximately nine feet; (c) A fiberglass extension ladder placed at the back porch of the house with its side rails approximately (18) inches past the roofline exposing employees using it to a fall hazard of approximately ten (10) feet; (d) A fiberglass extension ladder placed at the back of the house with its side rails approximately one foot past the roofline exposing employees using it to a fall hazard of approximately nine feet.
