29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.    a) West side of the commercial building: On or about December 2, 2021, and times prior thereto, employees were exposed to a fall hazard greater than 6 ft. to the ground below while installing the TPO on a flat roof without an additional form of fall protection with a warning line system.  b) Front side of the commercial building: On or about December 2, 2021, and times prior thereto, employees were exposed to a fall hazard greater than 6 ft. to the ground below while installing the termination bar without fall protection.

29 CFR  1926.502(f)(2)(v): The line was not attached at each stanchion in such a way that pulling on one section of the line between stanchions would not result in slack being taken up in adjacent sections before the stanchion tips over:  East side of commercial building:  On or about December 2, 2021 and times prior thereto, employees were exposed to a fall hazard, while performing roofing activities while using a warning line that had slack between stanchions.
