On or about 12/02/2021: Clark & Mott Construction employees were using a trailer mounted portable air compressor, connected to an air hose and a piece of thin pipe (lance) which had not included the use of safety clips, whip or other retainers to prevent the tool from becoming accidentally disconnected and creating a struck by hazard for employees utilizing the tool.

29 CFR 1926.302(b)(4):Compressed air, 30 p.s.i. or more, was used for cleaning purposes 269 Hamilton St. Worchester, MA 01604. On or about 12/02/2019: Clark & Mott Construction Inc, employees were using compressed air provided by a portable trailer mounted air compressor operating at greater than 30 PSI for clearing dirt in and around existing utilities, and had not provided effective chip guarding or other PPE to prevent employees being struck by flying debris.

On or about 12/02/2021: the employer ensured that ladders with structural defects were immediately tagged as defective or removed from service, exposing employees to falls. The ladder in use in the manhole had a bent rung and was not removed from service.
