29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Maintenance Shop Dayton Drill Press: On or about December 1, 2021 and times prior, an employee used a Dayton drill press to drill holes in parts. The rotating drill bit was not equipped with a guard to prevent employee exposure to flying chips and rotating parts.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) 8 inch Milwaukee Bench Grinder: On or about November 25, 2021 and times prior, employees used an Milwaukee 8" bench grinder to grind brackets and smooth metal edges. The grinder was not equipped with an adjustable work rest on the left side exposing employees to amputation injuries.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Maintenance Shop: On or about November 25, 2021 and times prior, an employee used a Milwaukee 8 inch abrasive bench grinder to grind down a bracket. The grinder did not have the adjustable tongue guard on the left side installed correctly resulting in a 1 inch opening between the abrasive wheel and the top of the grinder.
