Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached:  30  Amanda St., Springfield, MA: On or about December 3, 2021, an employee was observed engaged in residential roofing activities and the anchor for his personal fall arrest system had not installed according to manufacturer requirements. Anchor was installed with partially driven common nails and not attached with lag screws as required by the manufacturer.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

29 CFR  1926.502(d)(17):The attachment point of the body belt was not located in the center of the wearer's back:  30  Amanda St., Springfield, MA: On or about December 3, 2021, an employee was observed engaged in residential roofing activities more than feet above the next lower level and the lanyard of his personal fall arrest system was attached to the side D ring on his hip instead of the D ring located on his back and between his should blades as required.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
