29 CFR  1910.110(f)(2)(ii): Liquefied petroleum gas containers stored inside were located near exits, stairways, or in areas normally used or intended for the safe exit of people:   (a)Schoenhard's Custom Cabinetry, Inc. at 401 East 1st Street, Tea, SD 57064:  On or about and at times prior to December 3, 2021, the employer did not ensure that liquefied compressed propane containers were stored away from the exit path of employees, in that full LPG tanks were stored, unsecured less than three feet from an exit route.  This condition exposed employees to explosion and fire hazards.

29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a)Schoenhard's Custom Cabinetry, Inc. at 401 East 1st Street, Tea, SD 57064:  On or about and at times prior to December 3, 2021, the employer did not ensure lockout/tagout program was in place to prevent employee injury during maintenance or servicing of multi-energy source powered equipment to include but not limited to, the "Timesaver" machine and Optimat KD 68.  This condition exposed employees to caught-in and amputation hazards associated unexpected energizing, startup or release of stored energy.

29 CFR  1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph:  (a)Schoenhard's Custom Cabinetry, Inc. at 401 East 1st Street, Tea, SD 57064:  On or about and at times prior to December 3, 2021, the employer did not ensure that required powered industrial truck training was provided to operators.  Employees were operating the Daewoo model G25S, serial number 190943 forklift and had not received training prior to operating.  This condition exposed multiple employees to crushed-by and struck-by hazards associated with improper forklift operation.

This condition exposed employees potential shock and electrocution hazards related to inability to disable power during an emergency.  (b)Schoenhards's Custom Cabinetry, Inc. at 401 East 1st Street, Tea, SD 57064:  On or about and at times prior to December 3, 2021, the employer did not ensure unimpeded access to an electrical panel within the shaping area.  This condition exposed employees potential shock and electrocution hazards related to inability to disable power during an emergency.
