29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about November 17, 2021 the company did not establish an adequate program, including hazards of energy, control procedures, training and isolating machine(s) from energy sources from unexpected energy sources that could lead to injury. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about November 17, 2021 employee(s) performed set-up (servicing) on the Pleater machine were not required to take the following steps to control hazardous energy sources: 1) Know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy; 2) Turn off and shut down the machine(s); 3) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source; 4) Affix lockout or tagout devices to each energy isolating device; 5) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and de-energization of the machine or equipment have been accomplished. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
