29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. a. On or about December 2, 2021, at the back store warehouse, one 10-pounds, ABC dry chemical, portable fire extinguisher was not readily accessible. Physical and visual access to the extinguisher was obstructed by items laying on the floor. In addition, it was located in a very narrow space between a pallet rack and a wall. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c) and (d), respectively.

29 CFR 1910.178(p)(1): Powered industrial trucks found to be in need of repairs, defective, or in any way unsafe had not been taken out of service until restored to safe operating conditions: (a) On or about December 2, 2021, at the back store warehouse, a 5000 LBS capacity Caterpillar Powered Industrial Truck, Model GC25K, Type LP, was operational while having cracked and worn out tires, inoperable back-up alarm and strobe light, missing rear view mirrors and hydraulic fluid leak. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c) and (d), respectively.
