29 CFR 1910.1450(e)(3)(iii):The employer did not ensure fume hoods and other protective equipment were functioning properly and did not implement specific measures to ensure proper and adequate performance of such equipment. Placer Hall, USGS Labs, CSUS - The following inadequacies were noted in the U.S. Geological Survey water quality research labs. 1. Organic Chemistry Lab, #1107 - The fume hood averaged less than 100 FPM flow rate with both the sash fully open and with the sash adjusted to a 12" opening across the face. The hood velocity gauge was non-functioning. 2. Carbon Lab, #1110 - The fume hood averaged less than 100 FPM flow rate with both sash fully open and with the sash adjusted to a 12" opening across face. 3. Trace Metal, #1109 - The fume hood averaged less than 100 FPM flow rate with both the sash fully open and with the sash adjusted to a 12" opening across the face. 4. Biology Lab, #1111 - The fume hood averaged less than 100 FPM flow rate with both the sash fully open and with the sash adjusted to a 12" opening across face. *Note - Low flow rate alarms were observed going off during normal work use.

29 CFR 1910.157(c)(1): 29 CFR 1910.157(c)(1): Portable Fire Extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: Organic Chem. Lab, CSUS - A portable fire extinguisher was free standing on a lab counter and obstructed by lab materials. Lab employees use flammable materials and are expected to use the fire extinguishers in the event of a fire. Employees are exposed to fire hazards.
