Deemer Roofing & Sheet Metal Co. Inc. at worksite located at 220 Lincoln St. Port Clinton OH 43452, did not ensure employees were protected from falls while conducting residential roofing activities on the second level of a two story single family home, while exposed to a fall of 11 feet 3 inches to the ground below.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about December 7, 2021 the employer C.L. Deemer Roofing & Sheet Metal Co. Inc. at worksite located at 220 Lincoln St. Port Clinton OH 43452, did not have a fall protection training program in place to enable each employee to recognize the hazards of falling, and procedures to minimize these hazards. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness.

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  On or about December 7, 2021 the employer C.L. Deemer Roofing & Sheet Metal Co. Inc. at worksite located at 220 Lincoln St. Port Clinton OH 43452, did not provide a training program for each employee potentially exposed to fall hazards while using a portable ladder to enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards. Employees using the portable extension ladders to access the roof to perform roofing work were exposed to fall hazards of 11 feet 3 inches to the ground below.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about December 7, 2021 the employer C.L. Deemer Roofing & Sheet Metal Co. Inc. at worksite located at 220 Lincoln St. Port Clinton OH 43452, did not ensure the side rails of eight portable extension ladders were extended at least three feet above the landing surface. Four employees were exposed to falls of 11 feet 3 inches while using the ladder to access the second level.
