29 CFR  1910.119(d)(3)(i)(B): The employer did not include all equipment that was part of the process on piping and instrument diagrams.   (a) Anhydrous ammonia refrigeration system 2 - On or about 1/10/22, Refrigeration mechanics/maintenance  were exposed to a release of ammonia gas and liquid during maintenance activities  such as oil draining.   CD-2-4 BAC VXC-N-325 condenser and CD-2-3 BAC VAC-350 condenser were removed and replaced by an Evapco PMC-939E condenser (project completed on 5/19/21.)   CD-2-4 BAC VXC-N-325 condenser and CD-2-3 BAC VAC-350 condenser remain listed on the piping and instrument diagram, Engine Room #2 Condenser & H.P.R's, rev. 6/22/20.    Evapco PMC-939E condenser was not listed on the piping and instrument diagrams.  ABATEMENT CERTIFICATION REQUIRED

29 CFR  1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.(a) (LOCATION) (IDENTIFY SPECIFIC PROCESS OPERATIONS AND/OR CONDITIONS)   a) Anhydrous Ammonia System 1 Engine Room: On or about 12/21/2021, the employer did not document that the engine room emergency ventilation system complied with RAGAGEP, such as but not limited to, ANSI/IIAR 6 - 2019 Standard for Inspection, Testing, and Maintenance of Closed-Circuit Ammonia Refrigeration Systems, Chapter 12, Table 12.1 Emergency Ventilation Systems Inspection, Testing, and Maintenance Tasks, when inspections and tests were not performed on the emergency ventilation system equipment. 29 CFR  1910.119(j)(4)(iv): The documentation of each inspection and test that was performed on the process did not identify the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test:  a)  On or about 1/10/21, anhydrous ammonia refrigeration system #1 contains 40,393 pounds of anhydrous ammonia.   Refrigeration mechanics/maintenance were exposed to the release of ammonia gas and liquid when conducting daily checks and during oil draining.  The employer did not document that system piping has been inspected.  b)  On or about 1/10/21, anhydrous ammonia refrigeration system #2 contains 47,502 pounds of anhydrous ammonia.   Refrigeration mechanics/maintenance were exposed to the release of ammonia gas and liquid when conducting daily checks and during oil draining. The employer did not document that system piping has been inspected.   ABATEMENT CERTIFICATION REQUIRED
