29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees engaged in candy box making activities in the Box Plant - Line 17 at the Extension Edge Box Making Line were exposed to struck-by and amputation hazards in that the safety guard/gate was not fully enclosed allowing employees to step inside, to go around the interlocked door, to stand next to the glue machine and being able to reach into the spotter area to clear a jam box board while the machine was still running, was not shutdown. Employees were not provided protection from the point of operation, from struck-by hazards, from bypassing the safety gate/guard.
