29 CFR  1910.22(d)(1):  Walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition;  Inspections of walking working surfaces in the Walmart Auto Care Center were inadequate in that;  (a)The walking /working surface were not maintained in a safe condition.  (b) The Unilube Systems Pit Guard red grates walking working surface were not maintained in a safe condition in that the repairs being done during off hours such as, adjusting or replacing tension bolts that have moved out of position, were not inspected or approved.  (c) The Unilube Systems Pit Guard red grates walking working surface were not maintained in a safe condition in that employees were instructed to hit the red grates with a sledge hammer to knock them back in place so the Unilube Systems Pit Guard yellow grates could be closed completely after servicing a customer vehicle.
