OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards. Lincoln Center Manufacturing, LLC - Fulton, Ohio: On or about December 8, 2021, the employer did not ensure that a safety latch on the hook of the R&M (s/n 100959332) ten ton overhead hoist (underhung on bottom flange) was utilized. Employees were exposed to crush injuries in that the load and/or lifting devices could become removed from the hook as employees used the hoist with under-the-hook lifting devices to lift parts in the fabrication area. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Lincoln Center Manufacturing - Fulton, Ohio: On or about November 22, 2021, the employer did not ensure that the Yang CNC was adequately guarded to protect employees from pinch point, rotating part, and flying debris hazards. Employees were not prevented from accessing the hazard area at a time the machine operated.
