29 CFR  1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  Jobsite - On or about December 7, 2021, scrap lumber with protruding nails and all other debris from framing activities was not kept clear from the work area. Employees were exposed to slips, trips and puncture hazards in the work area in the front of the building as well as stairways used to enter the building.

29 CFR  1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:  Jobsite - On or about December 7, 2021, an employee was standing on a stack of several loose plywood sheets, at a height of approximately 20 feet, that was placed on the forks of the lull 644E-42 all terrain forklift which was not secured to the forks and did not provide fall protection.
