On or about 11/17/2021, at the site of 80-76 Springfield Blvd. Hollis, NY Employees working underneath of scaffolds were in danger of head injury from falling objects such as, but not limited to tools and materials from other employees who on the scaffolds. Protective helmets were not worn.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) A. On or about 11/17/21, at the site of 80-76 Springfield Blvd. Hollis, NY Employees working on the fabricated frame scaffolds were exposed to the hazard of falling approximately 18 ft. to the ground. The working platforms were installed with only two scaffold planks.

On or about 11/17/21, scaffolds at the site of 80-76 Springfield Blvd. Hollis, NY Employees working from tubular welded frame scaffolds were exposed to a fall hazard of approximately 18 ft. while applying stucco to face of 2-story with an attic house.
